Growing home health agency seeking dynamic LPN intake coordinator who will provide general office administrative duties. Someone who is not afraid of a fast-paced environment and willing to help others when the need arises. This position includes, but is not limited to, answering phones, managing fax communications, entering patient referrals into the EMR system, assisting with coordination of services, creating, modifying and copying forms as needed, filing, distribute Medicare paperwork, tracking physician orders, assembling admission packets, assisting with quality assurance activities and regulatory compliance. LPN INTAKE COORDINATOR QUALIFICATIONS:
- Possess a valid Driver’s License and reliable transportation.
- LPN License.
- Minimum of 1 year of Home health experience required.
- Experience scheduling Home health referrals, preferred.
- Proficient in Microsoft Office Suite applications Word, Excel, etc.
- Available to be on call – ROTATION
