JOB PURPOSE:
The purpose of this position is to provide non-medical case management services provide guidance and assistance in accessing medical, social, community, legal, financial, and other needed services related to benefits and entitlement counseling, and/or re-entry planning and providing psychosocial support services and counseling with clients to improve their health status.
SUPERVISORY AUTHORITY: N/A
NATURE AND SCOPE:
Coordinates and provides psychosocial services to families with HIV positive women, men, and children from birth to adolescence. In contact with a variety of Howard University and Howard University Hospital faculty, students, residents, and support/staff in various departments and community-based agencies and services.
PRINCIPAL ACCOUNTABILITIES:
- Conducts outreach/intake screening and follow-up interviews with families to develop information and assessments of medical/functional status; psychological; educational, financial; and family support needs.
- Develops a comprehensive care/service plan, including follow-up monitoring and periodic reassessment and plan modification, as appropriate for each child and family member.
- Provides consultation services to the medical and paramedical staff on the eligibility of the early intervention services for infants and toddlers.
- Provides HIV family-centered services for pediatric and obstetric inpatients and outpatients from a variety of settings, as appropriate, i.e., hospitals/clinics, private physicians, residential/ day care programs, community agencies, WIC centers, and government as well as private sector services.
- Prepares required documentation such as monthly progress reports and statistics, and reports to the coordinator and the director.
- Participates in the HUH C.A.R.E.S. administrative decisions regarding services, planning, structure, policy, procedure, etc.
- Prepares reports and provides data to the director as needed for planning and accountability.
- Represents HUH C.A.R.E.S. in meetings and provides coverage for the director as delegated.
- Stays abreast of developments in the public and private health care field, focusing on legislation, policies, procedures, programs, and problems.
- Maintains a good working relationship with community resources and shares information with staff. Interprets family-friendly and child-sensitive hospital, departmental, and C.A.R.E.S. policies and procedures, services, and programs to other staff, when indicated, and also to outside agencies.
- Assists in the implementation of family-centered treatment adherence.
- Promote adherence to the Health Sciences Compliance Program, the Howard University Code of Ethics and the Health Sciences Standards of Conduct.
- Attend annual and periodic mandatory Compliance Program training, including the Health Insurance Portability and Accountability Act (HIPAA) Privacy training.
- Participate in activities that promote adherence to Federal healthcare program requirements.
- Actively participates in Health Sciences Compliance Program activities. Adhere to the requirements of the HIPAA Privacy Policies and Procedures.
- Maintain confidentiality of patients, their families, and staff.
CORE COMPETENCIES:
- Skill in the operation of computers, related software applications (word processing, spreadsheet, database, graphic presentations, information systems, etc.) and standard office equipment.
- Working knowledge of motivational strategies, materials development, training, and consultation required.
- Thorough knowledge and understanding of health behavior change theories and their Application.
- Strong knowledge base or clinical experience in HIV.
- Communicate effectively orally and in writing with a diverse population.
- Must be detailed oriented and organized.
- Ability to exercise discretion and ensure a high-level of confidentiality.
- Ability to establish and maintain effective and harmonious work relations with faculty, staff, students and the general public.
- Exceptional customer service and phone etiquette.
- Ability to maintain effective and organized systems to ensure timely patient flow.
- Ability to write and keep accurate records.
MINIMUM REQUIREMENTS:
Bachelor degree in health or a human services related field. A minimum of 2 years working with persons with or at high risk of HIV infection. Case Management experience. Bilingual (Spanish) a plus.
