Community Health Worker – Hartford – Full Time

The Ryan White A Community Health Worker will provide preventive outreach, health education and linkage to services through the Hartford Youth HIV Identification and Linkage Consortia, in a myriad of settings including schools and in the community. 

  • Perform referrals and assists clients in linking with community support systems and agencies. 
  • Participate in administrative and clinical case conferencing for clients/patients with medical staff/medical case managers in order to address client’s/patients medical and psychosocial needs. 
  • Performs data entry tasks for evaluation purposes.
  • Monitor client progress within the health and community support systems. 
  • Provides education and training to residents/individuals in their respective community, works as part of the multidisciplinary team to prepare and implement plans, (example for HIV Pediatric and Youth program, Recovery Support Services; and general health awareness. Completes all required documentation. 
  • Participate in (in person/Virtual) community and planning meetings such as the Ryan White Planning Council and CT HIV Prevention Council, OCCH community meetings. 
  • Other duties as assigned.

Education Required: High school diploma or GED. 

Experience Required: 1 year experience working in one of the following: Medical case management, community health experience, community organizing/mobilization, faith based community programs, or worked with or experienced children/youth/individuals impacted by social determinants of health or chronic illness/substance/recovery programs. 

Experience Preferred:

  • Living and/or working in an urban setting
  • Ability to work with diverse client population
  • Ability to read and write; bilingual English/Spanish preferred 

Knowledge of: Community assistance resources 

Skilled at: Oral and written communications; interpersonal communications

Ability to: Work with diverse client population, read and write (English/Spanish if required), Flexible schedule: work evenings/weekends as needed
