Job Description:
To develop, implement and coordinate discharge planning activities for high-risk and complex patient cases. Ensures safe, timely and effective discharge from hospital setting to alternative levels of care. To provide in-depth psychosocial assessments and effectively communicate discharge plan of care with healthcare team members, patients and families.
1. Manages discharge activities for complex cases to reduce length of stay and maximize reimbursement. Seeks untapped resources to support the needs of high-risk and complex care patients and their families.
2. Conducts psychosocial patient assessment to determine patient needs and barriers to disease management of continued recovery. Documents findings in the clinical record according to established standards.
3. Implements appropriate social service interventions to address identified patient needs. Maintains a working knowledge of resources available. Provides and documents relevant patient/family education according to policy.
4. Leads the complex discharge committee facilitating an interdisciplinary discharge approach involving physicians and social services director.
5. Provides financial oversight for the discharged patient fund. Reconciles account and promptly explains any variances.
6. Serves as a preceptor for new social workers. Develops orientation content and core competencies.
7. Provides post graduate continuing supervision for LCSW applicants.
8. Performs other duties as assigned.
Master’s degree in Social Work and three (3) years of related experience required. Work experience may NOT substitute for education requirement. Currently licensed as a Licensed Independent Clinical Social Worker (LICSW) by the Alabama State Board of Social Work Examiners or currently licensed as a Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required.
