We are currently in need of a Full-Time Chaplain. 40 Hours Per Week in Milwaukee and Waukesha Counties. Make a difference in people’s lives during their end-of-life journey.
Chaplains act in coordination with the interdisciplinary group (IDG) to identify and meet the spiritual needs of hospice patients and families, including adjustment to living with a terminal illness, grief and loss issues, and the dying process.
Job Duties:
Complete spiritual assessment within 5 calendar days after initial admission to hospice
Establish care plan to meet spiritual support needs as identified by the patient and family
Provide bereavement support to patients and families pre-death and post-death as grief support
Provide support to patients and families for spiritual and/or faith rituals
Refer or coordinate care with patient and family identified spiritual community
Refer to local clergy, pastoral care provider, or other spiritual provider as requested by patient and family
