Job Overview:

Provides comprehensive case management to 50+ homeless mentally ill consumers. Completes Intakes, outreaches new consumers at known homeless locations, makes hospital and home visits, facilitates support and educational groups, develops service plans, provides supportive counseling, accompanies consumers to appointments as needed, drives consumers to appointments in agency vehicles, makes referrals, links consumers to needed services, enters data into HMIS daily, and documents all work in consumers electronic medical records.

Qualifications:

Required:

• BA or BS degree in Social Work, Psychology, Sociology, Behavioral Science, or Social Services
• At least one year of related working experience with adults in behavioral health.
• Valid, unrestricted NJ Driver s License required.

Preferred:

• Experience with the homeless preferable.

Scheduling Requirements

• 8:00 am – 4:00 pm
• Monday to Friday
• 40 hours/week
• Full Time

Pay Range:

$21.59 minimum to $26.53 maximum Hourly Rate
