Job Overview

The Social & Health Equity Navigator is responsible for working directly with AltaMed patients to address social factors and conditions that impact health outcomes, by assisting in reducing barriers and social determinants/drivers of health (SDOH) issues with the ultimate goal of improving the patient’s health, health care access, and quality of life. The Social and Health Equity Navigator conducts and reviews results of SDOH assessment/screeners to determine patient needs and identifies critical resources to link patients to services. The Social & Health Equity Navigator provides patients one on one support in coordinating care activities, support and assist patients to navigate through all the steps in obtaining the necessary and appropriate care, while documenting all efforts within the organization’s electronic health record (EHR) system. The Social and Health Equity Navigator builds strong relationships with patients in order to help them stay engaged in their medical care, and teach the patient how to utilize resources on their own to use as future tools. The Social & Health Equity Navigator will create protocols and workflows for our SDOH program, complete data entry and reporting requirements for SDOH, assist with SDOH patient registry and referrals, and assist with managing SDOH resources.

Minimum Requirements

1. High School diploma or GED required; Bachelor’s degree preferred in areas related to public health, social work or behavioral sciences or an equivalent education and demonstrated experience in the social determinants of health.

2. A minimum of two years of work experience in health, social or community services preferred.

3. English/Spanish required due to population served.

4. CPR/First Aid certificate preferred.

5. Proficiency in MS Office applications, including MS Work, MS Excel and Outlook required.

Salary Range

$24.00 – $24.00 hourly
