General Summary of position
Assesses, plans, implements, and evaluates the quality of care to patients during episodic and chronic visits. Assists physicians(s) in all diagnostic and therapeutic procedures, and provides education, counseling, and support to patients and their families.
Primary Duties and Responsibilities
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations. Participates in policy and procedure development and training.
Assesses the educational needs of patients related to their condition. Develops and/or implements a health education plan for each patient/family unit and for groups of patients.
Assists physician(s) with data collection.
Collaborates as part of the primary medical care team to formulate and document ambulatory care plans for patients and families.
Communicates patient’s clinical information clearly and accurately in verbal and written format. Assures that medical record is current, accurate, and contain appropriate documentation.
Completes nursing assessments and conducts ongoing assessments of patients and families. Evaluates patient progress, conveys information to the physician and coordinate appropriate follow up actions.
Ensures that laboratory specimens are properly labeled, the necessary paperwork is completed, and specimens are sent to the appropriate laboratory. Documents and files all laboratory results.
Implements medical orders and the primary medical care plan for each patient. Dispenses, monitors, orders, and administers (by mouth, injection, or IV) in accordance with the physician’s plan of care. Performs venipuncture to obtain blood samples for testing and other duties within the standards of practice.
Participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and hospital in community outreach efforts.
Performs other duties as assigned. May serve as the primary backup for the duties and responsibilities of other positions at the site.
Prepares patients for physical examination and/or specimen collection. Assists physician(s) with invasive and noninvasive procedures and examinations.
Processes telephone inquiries, performs telephone triage and documents patient phone calls.
Records services and diagnosis on patient’s encounter form to facilitate appropriate billing.
